Centreville Family Dentistry employs trained Treatment Coordinators to maximize each patient’s insurance plan. Each plan is as unique as you are. We are as committed to assisting in the insurance process as we are to providing you the highest quality dentistry. We are happy to submit all insurances for you.

The CareCredit healthcare card is designed to help you finance your health and wellness needs. With CareCredit, you can pay for treatments and procedures for you and your family right away and then make convenient monthly payments. In many instances our office will cover the interest for a time period.
Below are some documents that will need to be filled out prior to your initial visit. If you would like to save time, please feel free to print them to fill out at home. The Office and Financial Policy, Welcome Insurance Form, HIPAA, and Medical History Forms are required. All others may not apply unless discussed with our administrative staff. If you have copies of any existing dental records, we would appreciate that information to better serve you. Please also bring all information regarding health history and any medications currently taken. We look forward to seeing you in our office.
